If you are a member of Dr. Lovelace’s Direct Primary Care practice, the cost of the appointment and letter is free. If you are not a member of Derby City Direct Primary Care, the 30-60 minute evaluation and certifying letter is $250 and it is good for one year. This fee must be paid at the time of the visit. To get your Medical Cannabis certificate, you first have to be diagnosed with one of the qualifying conditions by a medical provider. The conditions that qualify for Medical Cannabis treatment: * Cancer * ALS or Lou Gehrig’s disease * Epilepsy * Intractable Seizures * Parkinson’s disease * Crohn’s disease * Multiple Sclerosis * Sickle Cell anemia * Severe and chronic pain * Post-traumatic stress disorder * Cachexia or wasting syndrome * Neuropathies * Severe Arthritis * Hepatitis C * Fibromyalgia * Intractable pain * Muscular Dystrophy * Huntington’s disease * HIV or AIDS * Glaucoma * Terminal Illness If you qualify, Dr. Lovelace (Kentucky licensed physician) will provide a written, signed certification that will expire in one year. The fee includes follow up appointments about questions on your medical cannabis certificate for up to one year. In the case you are denied, you will be refunded half of examination fee. Reasons for denial would include but are not limited to: suspected drug abuse, fraud and unqualified medical condition. WHAT TO BRING TO YOUR APPOINTMENT 1. Valid Kentucky Drivers’ License or Kentucky ID Card 2. Medical records with qualifying diagnosis, or ensure these are sent to Dr. Lovelace at least 48 hours prior to your appointment 3. Payment of $250 - All transactions for medical cannabis certificates must be paid in full at time of visit.
